KGMU 2025 Application Form

The candidates need to fill out the online application form and submit the duly filled form with the necessary details. The KGMU 2025 application form can be filled in two ways online and offline.

• Candidates need to keep a printed copy of the application form and all the important information, such as acknowledgement slip and fee payment Chalan, for future reference.

KGMU 2025 Eligibility Criteria 

To enter Degree courses at various undergraduate (UG), postgraduate (PG), and Doctoral level, in the field of Medicine & Health Sciences and Nursing candidates need to know the complete eligibility criteria.

Before applying to check the KGMU 2025 eligibility criteria of various courses.

• The aspirants seeking admission in the UG Programmes need to complete their 10+2 with a minimum of 50% marks in the relevant subject, from a recognized board.
• The aspirants seeking admission in the PG Programmes need to complete their graduation with a minimum of 50% marks in the relevant subject, from a recognized university/college.

Note: Aspirants are advised to thoroughly go through the eligibility criteria as mentioned by KGMU University for the course in which they wish to secure admission.

KGMU 2025 Application Process

The applicant must read the information brochure before filling out the KGMU 2025 Application form. As per the University guidelines, complete the application process to register the application successfully.

For PhD programs

• Visit the official website of KGMU 2025 and download the application form.
• Pay the application fee of INR 1500 through DD drawn in favour of “KGMU-PhD Program A/c” payable at Lucknow.
• Send the duly filled application form to the university address along with below-listed documents:

Check List of Enclosures:

• Draft of INR 1500.
• Attested Copies of Mark Sheets of Graduation & Post Graduation Degrees.
• Attested copy of Document in support of source of funding.
• Attested copy of the High School Certificate for the proof of Date of Birth.
• Attested copy of Caste Certificate (If applicable).

For Post MD-Ph.D. 2025 

The application forms can be obtained free of cost from the Department of Medical Education, KGMU, Lucknow. The application form can also be downloaded from the official website of the university. Submit the duly filled application form in person or by post along with DD of INR 1500 through DD drawn in favor of “KGMU-PhD Program A/c” payable at Lucknow to the university:

The application form should have the following enclosures:

• Three passport size self-attested colored photographs (to be affixed at the requisite places in the application form).
• Certificate from the Principal/Dean/Director/Registrar of the Institution from where the candidate has passed or appeared in MD/ MS examination, specifically mentioning the subject and the year of passing or appearing in the examination.
• Aspirants can pay the application fee by Credit Card/Debit Card/Net banking/E-Challan. In the case of E-Challan, the application fee can be paid in any branch of the Indian Bank.
