• Attested copies of the Diploma in General Nursing and Midwifery
• Attested copy of marks sheet of G.N.M 1st, 2nd & 3rd Year
• Relieving Cum Character Certificate from the School/ College last attended
• Attested copy of marks sheet certificate of 10+2 or equivalent examination
• Attested Copy of RN/RM
• Three recent passport-size photographs duly attested with name and father’s name written on the backside
• Punjab Domicile certificate / bonafide resident of Punjab certificate
• Affidavit
• Gap Year Affidavit (if applicable)
• Experience Certificate (if any)
• Attested copy/ copies of the certificate(s) of extra co-curricular activities, if any
• N.O.C., from the employer, if applying through the proper channel
• Two self-addressed envelopes stamped worth Rs. 30.00 each
• Copy of Aadhar Card

• The application fee of Rs.2000/- can be paid through offline mode.
