NEET (UG) Application Form 2019

Candidates will receive online the application form on the official website of the driving authority. The NEET (UG) 2019 Registration form will be launched as of the second week of February 2019.

Steps to Fill Application Form

Candidates have to follow the given five steps to fill the application form.

Step 1: Fill the Online Application Form and Note Down the Registration Number.
Step 2: Upload Photo, Signature, and Right Hand Index Finger Impression
Step 3: Make Payment of Fee by prescribed modes.
Step 4: Print Confirmation Page after Successful remittance of fee.

Step 1: Fill the Application Form

  • Go the official portal of the conducting authority (Link will be given above).
  • Click on “Apply Online” link.
  • A candidate has to read all the instruction and then click on “Proceed to Apply Online NEET (UG) (UG) 2019”.

Step 1.1: New Registration

  • Complete the registration form carefully.
  • Candidates must complete their name, mother’s name, father’s name, category, date of birth, gender, nationality, eligibility status (for 15% All India Quota), mobile phone number, email address, number of Aadhaar card, etc.
  • After completing all the details, there will be two links “Preview and next” and “Reset”.
  • If the candidate is satisfied with the complete information, he must click on “Next”.
  • After clicking on Next Button, the information submitted by the candidate will be displayed on the screen. If information correct, click on the “Next” button, otherwise go for the “Back” button.
  • Candidates may note down the registration number for further procedure.
  • Now choose the secure password and re-enter the password.
  • Choose the security question and the thread response.
  • Enter that the OTP will be sent to your mobile phone number.
  • Present the button.

Step 1.2: Login & Application Form Filling

  • Log in with your Registration Number and password.
  • Fill in the personal data.
  • Enter the place of birth.
  • Choose the middle of the questionnaire.
  • Choose the examination centers.
  • Fill permanent address.
  • Fill the correspondence address.
  • Fill Details (qualification, occupation, annual income) of parents and guardians.
  • Choose the option for dress code.
  • Enter security pin & click on save & draft.
  • Now click on preview and submit.
  • Now, review your entries.
  • Then. click on Final Submit.

Step 2: Upload Photo and Signature

  • Step 2 for images upload will appear on the screen.
  • Now, click on the link for Upload photo & signature.
  • Load the scanned images.
  • The candidate must have scanned images of his last photograph (size 10 Kb to 100 Kb.
  • Signature (size 3 KB to 20 KB) in JPEG format only.

Step 3: Fee Payment

After downloading the images, the candidate will automatically go to the link for payment of fees.
A candidate must follow the instructions and send the application fee.

  • Choose the bank to make the payment.
  • Go for the payment.
  • The candidate can pay the fees via a debit/ credit/net banking / electronic wallet (CSC) card.

Step 4: Print the confirmation page

  • After the payment of the fee, a candidate may take the impression of the confirmation page.
  • Candidates may keep at least three copies of the confirmation page.


You must keep a copy of the self-declaration generated by the system regarding J & K candidates who have opted for seats of less than 15% of All India Quota.
